What repo permissions are needed to create a Review App on Heroku? #124866

I am doing work for a client and trying to use Heroku review apps to test and then QA. I believe I should be able to manually create a review app through the Heroku pipeline dashboard when I see an Open PR listed. However, I don't see any open PR's listed, even though there is one. I see the message: "There are no open pull requests on [client/sample-app]"
When I tried to authorize the sample-app, I seems I do have permission, what permission is lacking?
I've tried Googling different things, but I'm stuck.
There is a repo: sample-app
There is a user: client
There is a user: dev
Client gave dev GitHub access to repo sample app
Client has authorized Heroku pipeline to connect to sample-app
Dev has Heroku permissions of "deploy" in the pipeline
When Dev tried to authorize sample-app to the pipeline, it did not appear as an option
I can hop on a call with a client and just ask them to try a bunch of things on a screen share, but it would be great to know ahead of time and to understand. And also have a reference for others having this problem.
